GENERAL INSTRUCTIONS
You are included in the Settlement Class if you received a Notice Letter the Medical Associates of the Lehigh Valley (“MATLV”) notifying you that your private information was compromised in the Data Security Incident.
Excluded from the Settlement Class are: (a) all persons who are employees, directors, officers, and agents of MATLV, the Judge assigned to the Action, and that Judge’s immediate family and Court staff.
Data Security Incident is the targeted cyberattack on the MATLV’s computer systems that occurred on or about July 3, 2022, in which certain files that contained private information were accessed.
COMPLETE THIS CLAIM FORM IF YOU ARE A CLASS MEMBER AND WISH TO RECEIVE ONE OF THE FOLLOWING SETTLEMENT BENEFITS.
AVAILABLE BENEFITS
Class Members may claim for ONE of the following benefits.
OPTION 1: CREDIT MONITORING AND INSURANCE SERVICES. All Class Members may claim 12 months of Credit Monitoring and Insurance Services provided by a credit monitoring bureau. This protection includes up to $1 million in identity theft insurance.
OPTION 2: DOCUMENTED LOSS PAYMENT: Class Members who suffered an actual, documented monetary loss as a result of the Data Security Incident may claim reimbursement for the loss. This reimbursement is capped at $5,000 per Class Members. Documented Loss Payment claims must include:
- a valid Claim Form electing to receive the Documented Loss Payment benefit
- an attestation regarding any actual and unreimbursed Documented Loss made under penalty of perjury
- Reasonable Documentation that demonstrates the Documented Loss to be reimbursed pursuant to the terms of the Settlement
You must submit documentation, such as receipts, to verify the costs you incurred. You may submit “self-prepared” documents to add clarity or support to other submitted documentation, but self-prepared documents by themselves are not sufficient to file a valid claim.
OPTION 3: CASH FUND PAYMENT. Class Members may choose to receive a pro rata payment from the Settlement Fund. The specific amount of the Cash Fund Payment will depend on the number of Class Members who claim other benefits, and the litigation and administration fees, costs, and expenses approved by the court.
To start your online claim form, you can find your Unique ID and PIN on the Notice that was sent to you. If you are unable to locate your Unique ID and PIN, please contact us by emailing info@MATLVSettlement.com. You will be asked to provide the Class Member’s full name and mailing address.
You may also download a Claim Form here, and submit it by U.S. mail to: MATLV Data Security Incident Settlement, c/o Settlement Administrator, PO Box 25232, Santa Ana, CA 97299. An electronic image of the completed Claim Form can also be submitted by email to info@MATLVSettlement.com.
The deadline to submit a Claim Form online is April 6, 2025. If you are mailing your Claim Form, it must be mailed with a postmark date no later than April 6, 2025.
